This is the final reminder that applications or waivers for group medical coverage must be complete and in the insurer's hands no later than Monday, April 28 for processing. Waivers can be faxed to the office any time, but applications need to be sent directly to the PA address with a void check, check for first and last month's premiums, application, union form, EFT, and the student form by the deadline.
